Office Policy
Booking Instructions
Patients are advised to call their family physicians office to book medical appointments. We suggest that you make your appointment early to avoid running out of medications or missing a deadline for an application/form that requires a medical opinion.
Our office fields a high volume of phone calls each day. Please understand that our staff are very busy and are doing their best to help everyone. Phone lines are generally most busy first thing in the morning and directly following the lunch break. It may be easier to reach your physicians office outside of these times.
If you strictly want to cancel a booked appointment, you may leave a message on our cancellation mailbox, which is monitored regularly, by calling 905-985-2985 ext 6224. If you would like to update your health card information, you may leave a message on our Health Card Update line at 905-985-2895 x5425.
Cancellation Policy
Our office policy is to require at least 24 hours notice of cancellation of a booked appointment. Failure to provide adequate notice may result in a missed appointment fee from your physician.
Regular Appointment - $30.00
Missed Physical Appointment - $60.00
For your convenience, you may call our cancellation voicemail box and leave your name and date of your appointment that you would like to cancel by calling 905-985-2895 x6224.
Uninsured Billings
Did you know that OHIP does not pay for all of the services your doctor provides to you?
Physicians can request payment from patients for the services that are not covered by OHIP.
Listed below are some examples of physician services that you can be charged for by your doctor:
NON-MEDICAL SERVICES
(some exceptions may apply)
- Transferred copies or a summary of your medical records, at your request, when changing doctors or when your doctor is relocating or leaving the practice.
- Single chart - $40.00
- Family Rate - $50.00 - Writing reports and filling out forms. Forms start at $20.00
Examples of such forms include:
- Licensing/insurance forms
- Federal Government forms
(e.g., Canada Pension Plan, immigration, etc.)
- Sick or back-to-work notes
- Medical legal reports
- Employer-requested reports
- Recreational camp forms
Based on the complexity of the form
Work/Sick notes - $20.00 - Drivers Physical (includes examination & completion of form) - $160.00
- Camp/School Physical (includes examination & completion of form) - $63.00
- CAS - Application to become a foster parent - $47.00
- Missed appointments without sufficient notice.
- Missed regular appointment - $30.00
- Missed Physical Appointment - $60.00
SOME MEDICAL SERVICES THAT ARE NOT COVERED BY OHIP
(some exceptions may apply)
- Cosmetic procedures (including some related consultations and diagnostic tests).
- Circumcision - $165.00
- Sclerotherapy - $168.00
- Cryotherapy - $24.00
- Acupuncture.
- Pre-departure travel medicine services for the purpose of traveling outside Canada
- Assessment and Injection of travel related vaccine - $44.00*
- Injection of Vaccine only - $12.00*
(*patient is responsible for the cost of the serum) - Routine eye examinations for patients who request more than the regulated amount covered by OHIP.
- Examinations/diagnostic tests required for the completion of documents such as reports, forms and licenses.
- Medical advice over the telephone or call-in prescription renewal orders to the pharmacist.
Speak to your doctor about other healthcare services that are not insured by OHIP, and of the fees you may be charged.
Payment for these services is required at the time of the service and can be made by VISA, Mastercard, Debit or cash.
Health Cards
Did you know...
You are required to show your valid health card at each visit with the doctor??
Did you know...
Some health cards expire? The green Ontario health cards are only valid for 5 years from the time of issue?
Did you know...
If you move, or loose your health card you should notify OHIP immediately? Your health coverage could lapse if OHIP is not able to contact you for some reason.

OHIP requires you to present 2 pieces of ID when renewing your health card? (One of them must be photo ID)

You must be physically present in Ontario 153 days in any 12-month period to maintain valid Ontario health coverage?
